Clear expectations make for a better experience for everyone. Please read through these policies before your first visit.

This practice is currently operating on a cash-pay basis. All payments are due at the time of service. Cash, checks, credit cards, FSA, and HSA cards are accepted.

A credit card is required on file at the time of booking to hold your appointment. Cancellation fees, when applicable, are charged to the card on file.

Upon request, a detailed receipt (superbill) can be provided after your visit. You may submit this to your insurance company for possible out-of-network reimbursement — please contact your insurer directly to confirm your coverage and benefits before your appointment.

Insurance credentialing

I am actively credentialing with Moda Health and Blue Cross Blue Shield. If you carry one of these plans, please ask about my current credentialing status when you reach out.

CANCELLATION & RESCHEDULING

Cancellation Policy

Because your appointment time is held exclusively for you, I ask for adequate notice when plans change — this allows me to offer that time to another patient in need. Cancellation fees are charged to the credit card on file.

48+ hours before your appointment

No charge. You're welcome to cancel or reschedule at no cost.

24–48 hours before your appointment

50% of the scheduled service fee will be charged.

Less than 24 hours before your appointment

100% of the scheduled service fee will be charged.

No-show (no contact made)

100% of the scheduled service fee will be charged.

New Patients — Courtesy Waiver

First-time patients will receive one courtesy cancellation waiver, no questions asked. I simply ask that you let me know as soon as possible so I can make your spot available to others.

Illness or Emergency

Life happens. If you are unwell or facing a genuine emergency, please reach out as soon as possible and I will do my best to work with you. Your wellbeing comes first.

APPOINTMENTS

Late Arrivals

If you arrive more than 15 minutes late, I may not be able to see you that day and the appointment may be treated as a late cancellation. If time allows, I will do my best to complete a shortened treatment — however, the full session fee will apply.

Please reach out as soon as possible if you're running behind so I can plan accordingly.

PROVIDER CANCELLATIONS

If I Need to Cancel

In the rare event that I must cancel an appointment due to illness, a family emergency, or circumstances beyond my control, you will be notified as early as possible. Provider-initiated cancellations are never charged a fee. I will make every effort to offer a rescheduled appointment within the same week whenever possible.

COMMUNICATIONS

Phone & Email

Brief phone consultations between scheduled appointments are provided at no charge. A consultation fee may apply to after-hours calls and to calls exceeding five minutes.

Email is intended for scheduling purposes only. I do not provide 24-hour emergency services. In the event of a medical emergency, please call 911 or proceed to your nearest urgent or immediate care center.
